Spravato for TRD - A New Path When Antidepressants Fail
Spravato for TRD - A New Path When Antidepressants Fail

Living with depression is exhausting enough without the frustration of treatments that simply do not work. You might have tried pill after pill, waited weeks for results, and still felt stuck in the same gray fog. This is known as Treatment-Resistant Depression (TRD). It is a heavy burden to carry, but you do not have to carry it forever.

Spravato® offers a different approach. This FDA-approved prescription nasal spray is designed specifically for adults who have not found relief with standard oral antidepressants. It works differently than the medications you have likely tried before. At MindBodyPinnacle Health, we guide patients through this process every day at our clinics in Laurel, MD, and Alexandria, VA.

Here is what you need to know about eligibility, what a visit looks like, and how we keep you safe.

Understanding Spravato and How It Works

Most standard antidepressants target serotonin, norepinephrine, or dopamine—chemicals in your brain that affect mood. Spravato targets the N-methyl-D-aspartate (NMDA) receptor instead. This receptor plays a key role in synaptic plasticity, which is essentially the ability of your brain to adapt and form new connections.

By targeting this different pathway, Spravato can sometimes offer relief when other medications have failed. It is used alongside an oral antidepressant, not as a replacement, to help give your brain the support it needs to reset. According to the National Institute of Mental Health (NIMH), treatments targeting the glutamate system (like esketamine) represent a major shift in how we approach difficult-to-treat depression.

    Who Is Eligible for Spravato?

Figuring out if you qualify is the first step. Spravato is not a first-line treatment; it is reserved for specific situations where other options have been exhausted.

You typically need to meet a few key criteria to be considered for this treatment:

    • Diagnosis: You must be an adult with a diagnosis of Major Depressive Disorder (MDD).
    • Treatment Resistance: Your depression must be considered “treatment-resistant.” This usually means you have tried at least two different antidepressants at adequate doses for a sufficient time without significant improvement.
    • Current Medication: You must be willing to take an oral antidepressant daily alongside your Spravato treatments.

    Who is it NOT for?

There are some medical conditions that might make Spravato unsafe. According to the FDA label, this treatment is contraindicated if you have:

    • Aneurysmal vascular disease (including thoracic and abdominal aorta, intracranial, and peripheral arterial vessels).
    • Arteriovenous malformation.
    • A history of intracerebral hemorrhage.

These conditions make the temporary rise in blood pressure caused by Spravato dangerous. We can help determine your eligibility during a consultation. We offer telehealth evaluations for patients in Washington D.C., Maryland, and Virginia to make this first step easier for you.

What Happens During a Treatment Visit?

The treatment experience with Spravato is quite different from taking a daily pill at home. Because of the way the medication affects you, it must be administered in a certified medical office. You cannot take the spray home with you.

Here is how a typical visit unfolds at our Laurel or Alexandria clinics:

    1. Arrival & Check-in: You will arrive at the office and get settled in a comfortable chair. A healthcare provider will check your blood pressure to make sure it is within a safe range before you start.

    2. Self-Administration: You will administer the nasal spray yourself under our direct supervision. The device is easy to use, and we will guide you through it.

    3. Observation Period: After you take the medication, you must stay in the clinic for at least two hours. This is a strict safety requirement. During this time, you can relax, listen to music, or read. Many patients find it helpful to bring headphones and a calming playlist.

    4. Monitoring: Our staff will check on you periodically to monitor your blood pressure and ensure you are feeling okay.

    5. Discharge: Once the observation period is over and your clinician clears you, you are free to go.

    Important Safety and Driving Rules

Safety is our top priority. Spravato can cause side effects like dissociation (feeling disconnected from yourself), dizziness, sedation, and vertigo.

Because of these potential effects, you are not allowed to drive or operate heavy machinery for the rest of the day after your treatment. You must arrange for a ride home. This could be a friend, a family member, or a ride-share service. You should plan to rest for the remainder of the day and get a good night’s sleep before you get back behind the wheel.

    The REMS Program Explained

You might hear us mention the REMS program. This stands for Risk Evaluation and Mitigation Strategy. It is a mandatory safety program required by the FDA to ensure Spravato is used safely.

MindBodyPinnacle Health and all our healthcare settings are certified in the Spravato REMS program. This means:

    • We are trained to monitor for specific side effects.
    • We report monitoring data to the FDA to help track safety.
    • Your pharmacy must be certified to dispense the medication to our office.

We handle the coordination to make it as smooth as possible for you. You can learn more about this program at the official Spravato REMS website.
