We are not part of the Department of Veterans Affairs. We provide independent clinical evaluations and documentation to support your claim. The VA makes all rating decisions and may still schedule its own Compensation & Pension (C&P) exam.
What is a VA DBQ?
A Disability Benefits Questionnaire is a standardized VA form that captures medical findings for specific conditions. Submitting a complete, accurate DBQ can strengthen your claim by aligning clinical evidence with the VA’s rating criteria. DBQs are available for many conditions, including mental disorders and PTSD (review). Some DBQs, like Initial PTSD or Traumatic Brain Injury, are restricted to VA examiners.
Where a DBQ fits in your claim
Initial or Increase claims: A DBQ helps present your current disability picture. It is not required, and it does not guarantee a rating.
Evidence package: Pair a DBQ with medical records, service treatment records, prior rating decisions, and (when appropriate) a strong nexus opinion.
C&P exams: Even with a private DBQ, the VA may still schedule an exam. If scheduled, you must attend.

What to Expect With MindBodyPinnacle Health
Step 1 — HIPAA‑secure intake
Secure forms, photo ID verification, and a release so we can review records you choose to share.
We’ll ask for service treatment records, prior C&P exams, rating decisions, private treatment notes, and symptom logs.
Step 2 — Clinical evaluation
One‑on‑one interview covering history, current symptoms, and functional impact at work, school, home, and in relationships.
Evidence‑based screening tools when clinically indicated (for example, PHQ‑9, GAD‑7, PCL‑5).
Step 3 — DBQ completion
A licensed clinician completes the appropriate DBQ(s), documents severity and occupational/social impairment, and includes required identifiers (license, NPI, specialty).
For conditions that require VA‑only DBQs (for example, Initial PTSD or TBI), we provide a detailed clinical narrative to support your record and can prepare you for any VA‑scheduled exam.
Step 4 — Quality check and delivery
Final review for internal consistency and completeness.
PDF delivery for your records and submission through VA.gov or your VSO/representative.
We do not submit claims on your behalf and we do not provide legal advice. Bring any paperwork to your VSO, accredited agent, or attorney.
